What Tax Documents Do I Need?
The following list of documents is provided to help you be ready for tax time.
Please note that not all items will apply to everyone.

General Documents
• Driver’s license or ID card
• Bank routing & account numbers for direct deposit
• 1095-A form (if ACA health insurance)
• Estimated Tax Payments—Dates and amounts paid
• Prior year tax return
• IRS Identity Protection PIN (if issued a 6-digit code)
FAMILIES WITH NEWBORNS:
• Child’s name, date of birth, SSN (for Trump Account registration)
FIRST TIME CLIENTS WITH CHILDREN:
• Copies of Social Security cards & birth certificates
• Proof of residence for dependents if claiming EIC
Income Documents
• Income from jobs: W-2s.
• Interest Income: 1099-INT.
• Dividend Income: 1099-DIV.
• Investments: 1099-B.
• Retirement Income: 1099-R.
• Social Security: SSA-1099.
• Income from state(s): 1099-G.
• Taxable alimony received.
• Rental property: income and expenses, property details.
• Income from sale of property—original cost and cost of improvements, escrow closing statement.

• Cancellation of debt: 1099-C.
• Other Income: jury duty, gambling winnings.
Credits & Deductions
• Student loan interest.
• Educator expenses.
• Deductible alimony paid.
• Education costs: form 1098-T.
• IRA / ROTH contributions.
• HSA / MSA contributions.
• Keogh, SEP, SIMPLE, and other self-employed pension plans.
• Medical and dental expenses.
• State & local income taxes paid.
• Real estate taxes paid.

• Vehicle license fee based on value.
• Home mortgage interest: 1098.
• Cash Charitable donations— organization, date, amount.
